Required Health Documents & Eligibility

To attend Westfield State University, you need to submit the following required health documents:

• TB Risk Assessment Form
• Immunization Requirements
• Health History
• Physical Exam (completed within the last 12-18 months)

Once complete, please upload the required documents HERE (preferred method). You can also mail the forms to Westfield State University Health Services at 577 Western Ave, Westfield, MA 01086, or fax them to 413-572-5545.

The deadline for submission of all student health records is July 1 for the fall semester or December 1 for students entering in the spring semester.

All full-time undergraduate students, regardless of health insurance, can utilize the Department's services. There are no fees charged for visits or medications dispensed/administered in Health Services.  

Medical records are strictly confidential. Information will not be released without written authorization from the student or a court order. Authorization forms are available at Health Services. See the University's notice of Privacy Practices.

The department does not provide sick notes. Students are encouraged to work directly with their faculty to address any attendance concerns.
